Registration

If you are being admitted to the hospital for an inpatient stay, please come to the registration desk at the Emergency Room/Outpatient entrance. The registration process takes 15 to 20 minutes, so please allow for this.
 
Bring with you:

• Your doctor’s signed orders
• All of your insurance cards
• Your driver’s license or photo ID
• All of your medications (or a complete list of them)
• Your living will or healthcare power of attorney (if you have them)

Surgical Registration

If you and your physician agree on the need for a surgical procedure, your doctor’s office will call KershawHealth to set a date. A few days before your scheduled surgery date, a representative will call you to get the necessary registration information. They will explain everything you need to know prior to your surgery.
 
When you come for surgery, you will still need to stop by the registration desk to sign your paperwork. If you pre-registered in person, you may go straight to surgery.
