Diabetes Management

When you first find out you have diabetes or are borderline diabetic, you’re very likely to have many questions about the best way to manage this disease and stay healthy. To get the help you need, ask your physician to refer you to KershawHealth’s diabetes education program.

The It’s Your Choice Diabetes Self-Management Program, recognized by the American Diabetes Association, is designed to help people, just like you, better manage this difficult disease. The program provides comprehensive education to:

  • Enable people with diabetes to make better lifestyle choices to manage their disease and prevent complications.
  • Encourage those at risk for developing diabetes to make better lifestyle choices to promote good health and prevent the onset of diabetes.
Services provided by the program include:
  • Individual assessment and one-on-one consultation with a diabetes nurse educator
  • Group classes taught by a diabetes nurse educator, registered dietician and other healthcare team members
  • Diabetes support group
  • Educational presentations to community groups
Topics that will be covered in your classes include:
  • Types and causes of diabetes
  • Medications
  • Nutrition management
  • Blood sugar monitoring
  • Acute and long-term complications
  • Managing sick days
  • Exercise and diabetes
  • Reducing stress and coping with diabetes
  • Setting realistic goals for better diabetes management

So if you’ve been diagnosed with Type 1, Type 2 or gestational diabetes, you may be eligible to participate in this program. A referral from your physician is required for the initial individual assessment, one-on-one consultation and group classes. Many insurance plans cover this program, and there may be funding available to those without insurance and unable to pay.

The Diabetes Education program is located at the KershawHealth Medical Center in Camden. 
Upon receiving a referral from your physician, the diabetes educator will contact you to schedule an appointment or you may call her at the number listed below.  When you come for your initial assessment, please stop at Registration.
